Long Term Care Coordinator

This is an administrative position in Otsego County’s Office for the Aging that involves the responsibility to oversee information and assistance provided to consumers that access long term care services.  The emphasis of the program is to explore all community-based alternatives to institutional type services with specific emphasis on self-determination, personal responsibility and cost efficiency. The range of the coordinator’s services will cover all segments of the population and will include constant and effective contact with hospitals, nursing facilities, adult homes, Certified Home Health Agencies, advocacy organizations, County departments and other long term care stakeholders.  The Coordinator will work under the superviĀ­sion of the Director of the Office for the Aging.  The incumbent may make field service visits as needed to assist or supervise staff.  Incumbent is responsible for all office superĀ­vision; does related work as required.

 

Candidates must meet the following qualifications:

 

(A) Graduation from a regionally accredited or New York State registered college or university with a Bachelor's Degree in human services, social work, psychology, health care administration, nursing or a closely related field and two years of experience involving the assessment of consumer needs or planning of consumer services.

 

Note: Additional education in a closely related field to the above areas may be substituted for the required experience on a year for year basis.
